Admission Procedures:


1. Secure application form from the Registrar.
2. Pay the non-refundable testing fee at the SJWMS  

     Cashier’s office.
3. Present the OR to the Registrar and a written

     assessment will be given.

4. Wait for the result of the assessment.

5. Submit  all the requirements for admission to the

     Registrar.

6. Make payment of the Enrollment fees to the cashier. 

7. Present OR to the registrar for final enrollment

    listing. 

 

 

REQUIREMENTS:

• Report Card-Form 138 

• Permanent Records-Form 137

• 2x2/1x1 ID Pictures (2 Copies each size)

• NSO Authenticated Birth Certificate 

• Certificate of Good Moral Character

• Alien Certificate for Foreign Nationalities and Study Permit

Additional Requirements for incoming Senior high School Students

• Grade 9 NCAE Results 

• Moving -Up Completion Certificate

• QVR Certificate (Students from Private Schools only)
